Every patient falls into one of two buckets first. Mechanism of injury (MOI) is trauma — some outside force did something to the body: a collision, a fall, a stabbing, a blast. Nature of illness (NOI) is medical — the problem started inside the body: a heart that's failing, lungs that can't move air, a brain that's seizing.
So which one is it? Scan for both. Don't lock in on the first thing you see.
Here's why that matters. A driver crashes into a pole. You could call that pure trauma and start working the crash. But what if he passed out first, and that's why he crashed? Miss the syncope, and you'll miss why he's about to pass out again in your ambulance.
That's why you always ask what happened first.
