Interfacility and critical-care transports hand you a patient whose plan is already set. Your job turns into monitoring and troubleshooting, and the exam is testing whether you can tell a patient problem from an equipment problem.
For any sudden deterioration in an intubated or ventilated patient, work the same sequence every time. Displacement of the tube, right mainstem or esophageal, dislodged during a move. Obstruction: secretions, a mucous plug, a kinked or bitten tube. Pneumothorax: rising pressures, unilaterally diminished or absent breath sounds, hypotension, distended neck veins. Equipment failure: disconnected circuit, oxygen supply, ventilator malfunction.
