Circulation Assessment: Pulse, Bleeding, Skin, and Perfusion

1. What circulation assessment actually checks

Circulation is not just a pulse check. The Instructional Guidelines break it
into four pieces:

1. Radial pulse present or absent — and if present, its rate and quality
2. Major bleeding — find it and control it now, not later
3. Perfusion status — skin color, skin temperature, skin moisture
4. Capillary refill — "as appropriate," in the Registry's own wording

That second item is the one candidates drop. You check a pulse, you skip the
bleeding sweep, and you've just run an incomplete primary assessment.
Circulation means pulse and hemorrhage and perfusion. Not just pulse.

Here's the physiology underneath all four checks. Perfusion is blood
actually reaching your tissues — delivering oxygen, picking up waste. When
perfusion fails, that's shock. So every one of these four checks is
really one question asked four different ways: is blood getting where it
needs to go? The pulse tells you the heart is moving blood. The bleeding
check tells you whether blood is leaving the system. The skin tells you
where the body is choosing to send blood. Capillary refill tells you how
well it's reaching the smallest vessels. You'll use that same frame in every
section below.
